How Senior Horizons works:
- Senior Horizons professionals begin with a comprehensive, in-home assessment of the older adult’s needs, strengths, and interests that culminates in a plan of support for the older adult.
- The social worker provides as much care management and supportive counseling as desired to help implement the plan of support and monitor the older adult’s overall health and well-being, recommending adaptations in support as needed.
- As an alternative to in-home services, we can provide consultation to families in our offices at four convenient locations.
